Lifeline Healthcare seeks a seasoned geriatric specialist to join our care management team. RESPONSIBILITIES: Bring your skills to our clients, greatly enhancing their quality of life. Work one-to-one with older adults in their homes and in facilities, developing and implementing their care plan. Conduct assessments, build and maintain excellent relationships with families and other professionals. Supervise caregivers in executing care plans and motivating clients. Join our highly knowledgeable, caring, and experienced team to develop yourself as a Care Manager. We are dynamic, skilled, and growing. REQUIREMENTS: Undergraduate degree required, masters degree preferred. Qualifying backgrounds include MSW, LCSW, social worker, registered nurse (RN), licensed vocational nurse (LVN), recreation therapist, Case Manager, Residential Care Facility for the Elderly (RCFE) Administrator, Activity Director, or Activities Coordinator. Diversified experience will help, especially in areas demonstrating self-initiative, independence, administrative, supervisory, and business skills. Experience in geriatric care and Alzheimer's Disease, including clinical knowledge of diagnosis. Own transportation. Organization and time management skills. Fast learner. Pursues excellence.
