Qualifications: LPN, RN, BSN, BSW, LBSW, LMSW, LISW, CCM, CMC, C-ASWCM, C-SWCM, A-SWCM, ACSW, CICSW, CLCP, BA, BS, BSG, CFLE, CLCP, GCS, GMS, GMHS, LCPC, LMFT, LPC, LMHC, LMHP, LPC, MA, RNCM, MSG , NBCC, SA, SWA. Candidate must have clinical experience in aging and medical terminology; goal should be to become a certified professional geriatric care manager. Must have: driver’s license, computer skills, dedication to working with and advocating for older people. Required to be on-call.
